camp registration form

This form can be used to register an Individual player or to indicate you will be registering as a team.  Team Registration form is required before Team registration is complete. Please follow the prompts to ensure proper registration. 

If registering an individual, after entering requested information hit "Submit" button to complete registration.

If registering a team,  individual player information will need to be listed on the Team Registration form. To submit, go to "Submit Camp Forms" page.
